To Request Copies of Records:
As we provide services at reduced cost, we do not maintain a full-time office staff for managing patient records and our records are not computerized. We are able to provide copies of records for a $5 fee to cover the cost of copying, mailing, and the time to locate the record. To request a record, please provide:
- store location and date of clinic attended
- owner's name
- pet's name
- address to which the record should be mailed
- $5 check payment made out to Veterinary Wellness Clinics
